Building connections for a healthier Nebraska.

Unite Nebraska is a Community Information Exchange (CIE) of health and social care providers sponsored by Nebraska Health Information Initiative (NEHII).

Partners in the CIE are connected through a shared technology platform, Unite Us, which enables them to send and receive electronic referrals, address people’s social needs, and improve health across communities.

As a Unite Nebraska partner, you can:

Easily refer and connect your clients to local services they need in the community.

Improve your clients’ health and well-being through strengthened collaboration with partners offering a wide array of services.

Track the outcomes of all referrals and services delivered for your clients.

Measure the impact of your organization and the services you deliver.

Improve organizational capacity through accurate referrals and access to a wealth of data on local service delivery.

"It's our honor and privilege to work with NEHII, who understands well the need for one holistic and integrated system to improve Nebraskans' health across the state. We are excited to provide the state with their first golden record on individual Nebraskan needs by combining clinical and social care data in one secure location."

-Taylor Justice, Unite Us President.

"Before, we would give out paper referrals and had no follow up tracking. Having this platform enables us to follow up with an agency if the client was not served and we can track how quickly the client was served, and if they did receive assistance. In this day and age with the pandemic, this platform provides more resources for clients and your organization."

- Michelle Rodgers, Program Manager at Heartland Hope Mission, a faith-based food pantry in the Omaha metro area that provides a week's supply of food, clothing, hygiene items and resources to the needy.
